Title: The reconstitution of work practices during a radical hospital transformation

Abstract: From an organizing perspective, Improving care provision in hospitals is particularly challenging due to the complexity of work processes and the diversity of diagnoses that need to be treated. With the goal of improving hospital care performance, the city of Montreal recently moved and consolidated several hospitals into a $3 billion tertiary care mega-hospital. In this presentation, I report on a 30 months field
study of this transformation process with a specific focus on the Neonatal Intensive Care Unit (NICU). We offer analysis regarding the challenges induced by the move, the adaptation to a new space, and the melding of units previously located into different hospitals. Using ethnographic approaches and focusing on work practices, we trace how the NICU staff reacted and adapted to the sweeping change. Our findings offer insight as to how long established and typically opaque work practices need to be changed as to ensure patient care and unit functioning. From this radical hospital restructuring, we gain insight as to how existing work practices are constituted and how they are  reconstituted given novel material and social foundations.
